Overview

From a blank SU-21 document, there are only three sections that you are required to complete. The following sections are the ONLY sections you need to fill out:

• 5 of 7

1. Complete Sections 2, 3, and 7

Section 2 of 7 requires that you fill out your identifying information.

• Please use your SSN if you do not know your 8 digit Stanford ID number.

Section 3 of 7 asks how you would like to receive your stipend.

• Department Mail Code: If you choose the this option, you will pick up your check from us when you to get Stanford.
• Direct Deposit: If you choose this option, you will have to fill out a second form. Please note that you either need to attach a void check or fill out your bank information on the form.

Section 5 of 7 asks for your citizenship status and/or visa information as well as your signature. Please do not type signature, you must sign via electronic signature. Instructions on how to do so below.

2. Sign via Electronic Signature

<aside> 📌 As a reminder, in order to process the award, your name cannot be typed. It must be signed virtually.

Windows

To electronically sign a PDF on Windows, open Adobe Reader and click “Fill & Sign” to add your signature.

Mac

On a Mac, open the PDF in Preview, click the “Show Markup Toolbar” button (if the Markup toolbar isn't showing), then click the “Sign” button.
